How to Add Notes or Captions to iPhone Photos (Permanent)

By ··5 min read

Adding notes to iPhone photos: use the Photos app markup (tap photo > Edit > Markup pencil icon to draw or add text), add to Apple Notes as attachment, or save to Nemos which auto-generates a searchable note from the photo content via OCR.

Method 1: Photos App Markup

Open any photo > Edit (top right) > Markup icon (pencil) at bottom right. Add text boxes, drawings, arrows, or shapes directly on the image. These appear when the photo is opened and export with the photo.

Best for: adding a label or annotation directly to a visual.

Method 2: Apple Notes Attachment

Create or open an Apple Notes note > tap image icon > select the photo. The photo appears embedded in the note. Type notes below or around it. Note syncs via iCloud and is searchable by text content in the note body.

Best for: structured notes where the photo illustrates a specific point you’re writing about.

Method 3: Nemos (Automatic OCR and Title)

Share any photo to Nemos via the share sheet (Photos > Share > Nemos). Nemos:

  • Reads all text visible in the photo via OCR
  • Auto-generates a descriptive title based on content
  • Files into Smart Spaces by subject
  • Makes every word in the photo searchable

The "note" is implicit: search any word visible in the photo and Nemos retrieves it. No manual labeling required.

Best for: screenshots of receipts, whiteboards, documents, handwriting, or any photo where the content should be searchable.

Context-First Photo Organization

iOS Photos app captions (accessible via Info button in iOS 17+) are one option. They're hidden unless you tap Info, don't surface in search meaningfully, and don't transfer reliably when photos leave Apple's ecosystem.

Nemos treats photos and context as a unified capture. Share any photo from Photos app → Nemos → add a voice note or text note. Now the photo and its context are stored together, searchable together.

Practical difference: search "plumber quote March" in Nemos and find the photo of the written estimate alongside the voice note explaining which repair it covered. In Photos, you'd search date or location but not the semantic content of what was photographed.

For memory preservation: photos of people, places, and moments gain searchability when captured with context notes describing what was happening and why it mattered.

Adding a photo to a note (the other direction)

Two things get searched with almost the same words, and most people arrive wanting the second:

  • A note attached to a photo — annotating an image you already have, which is what the steps above do.
  • A photo placed inside a note — what you want if you are writing and need a picture in the middle of it.

For the second: open the note, tap the camera button above the keyboard, and pick Choose Photo or Video to insert one from your library or Take Photo to add a new one. The image sits inline and moves with the text around it.

In Nemos the distinction does not arise: a screenshot saved to your library is already readable and searchable by the words inside it, and a note can hold images beside the text without either being an attachment to the other.

Native iOS steps are offline-first. Third-party app behavior depends on the app — any cloud dependency is flagged explicitly.
